I really believe what God is saying here is this, if you as a people are living completely perfect, sinless lives (which they won't and can't) then disease and sin will not have a place among you. I think (and I could be wrong) that disease and barrenness are not necessarily a sign of sinfulness among the person who is barren (look at Hannah in 1 Samuel for an example), but the sinfulness that exists among the people.
